防治隐患治理项目（第二次）招标公告
根据吉林省财政厅政府采购管理办公室下达的政府采购任务通知书，吉林省政府采购中心于2017年5月18日就吉林省安全生产监督管理局(本级）吉林省用人单位职业病防治隐患治理项目进行国内公开招标，因投标供应商不足三家而废标，现重新进行国内公开招标，邀请合格的投标人提交密封投标。

normal style='text-indent:24.1pt;tab-stops:156.0pt'>5.1 符合《中华人民共和国政府采购法》第22条规定的条件。
normal style='text-indent:24.1pt;tab-stops:156.0pt'>5.2 具备国家有关主管部门批准的制造（和/或经销）本标的的合法资格。
normal style='text-indent:24.1pt;tab-stops:156.0pt'>5.3 单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动。
normal style='text-indent:24.1pt;tab-stops:156.0pt'>为采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的供应商，不得再参加本采购项目的其他采购活动。
normal style='text-indent:24.1pt;tab-stops:156.0pt'>5.4 投标人须提供具有职业卫生技术服务机构乙级及以上资质（复印件，加盖投标人公章）。

七、招标文件的获取时间和方式：自本项目招标公告发布之日起至2017年06月05日 16:00（北京时间，下同），请有意投标的供应商自行登陆吉林省政府采购中心网站下载。
八、公告期限期限及确认参加投标截止时间：自招标公告上网之日起至2017年06月05日 16:00止。

normal style='text-indent:24.1pt;mso-char-indent-count:2.0; mso-outline-level:1'>1、首先登录吉林省政府采购中心网（www.jlszfcg.gov.cn），按照规定进行供应商注册登记，网上注册登记后，请携带相关材料到国投安信数字证书认证有限公司所在地办理CA认证。未进行网上注册并办理CA认证的供应商将无法参与吉林省政府采购中心组织的所有招标采购活动。”
normal style='text-indent:24.1pt;mso-char-indent-count:2.0; mso-outline-level:1'>2.供应商取得CA认证后，可登录吉林省政府采购中心网站“CA登录区”下载电子招标文件。招标文件由《通用部分》和《需求部分》两部分组成。供应商下载招标文件后，务必在规定的“确认参加投标截止时间”之前点击“投标”按钮确认参加投标才具有投标资格。如果供应商在规定的“确认参加投标截止时间”之前没有点击“投标”按钮确认参加投标，将失去参加本项目投标的资格。
