normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>1、投标人参加投标的意思表达清楚，投标人代表被授权有效。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>2、投标人均具有独立法人资格，按国家法律经营。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>3、投标人均持有建设行政主管部门颁发的企业资质证书及安全生产许可证；

4、投标人应具备以下资质：

投标人应具有承接本工程所需的市政公用工程施工总承包一 级或以上级别资质；

投标人拟担任本工程项目负责人的人员为：市政公用工程专业一 级或以上注册建造师；

注：建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。

根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函[2011]218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人应提供有效的二级建造师执业证书或省建筑执业资格注册中心“取得二级建筑师、二级结构工程师、二级建造师执业资格证书持证人员从业情况信息公开平台”上有效的《二级建造师执业资格证书持证人员从业情况信息公开证明》。

5、项目负责人持有在有效期内的安全生产考核合格证书（B类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。专职安全人员须具有在有效期内的安全考核合格证书（C类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。项目负责人和安全员不为同一人。

6、投标人自nth="1" Day="1" IsLunarDate="False" IsROCDate="False" w:st="on">2014年1月1日至今已完成过质量合格的类似工程业绩（类似工程是指中标金额不少于4000万元的市政工程业绩，完成时间以竣工验收时间为准，中标金额以中标通知书为准）。需同时提供中标通知书或免招标的相关证明、施工合同、竣工验收报告、工程竣工备案表或建设工程质量监督部门出具的工程竣工合格验收的证明。如以上资料不能证明业绩规模的技术指标（指面积、高度、跨度、管径等）的，须另提供可证明业绩技术指标的其他资料。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>9、资格审查前，投标人在广州住房和城乡建设委员会建立了企业诚信档案及拟担任本工程项目负责人为本企业诚信档案中的在册人员。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>10、未在以往工程中因不诚信行为或不充分履约行为被本项目招标人书面拒绝投标的。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：（1）项目负责人在任职期间不得但任专职安全员，项目专职安全员在任职期间也不得但任项目负责人；

normal style='text-indent:44.9pt;mso-char-indent-count:3.74;line-height:150%'>（2）上述投标人合格条件3至6项（除第5点需提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页外）的信息取自投标人在广州市房建和市政企业库内上传件的，投标人证明符合资审合格条件必须按招标文件nth="12" Year="1899" w:st="on">11.2.1的要求提供书面资料。评标委员会对该部分资料的审查将以递交投标文件截止时在广州市房建和市政企业库内上传件为依据。若招标人延长投标报名时间，以上资料以延长投标文件截止时在广州市房建和市政企业库内上传件为依据,投标人应及时维护相关信息，确保各上传件的有效性。

一、本公司保证投标报名材料及其后提供的一切材料都是真实的。

二、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录，本公司没有被人民法院列入失信被执行人名单。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后不转包或违法分包，在施工过程中，严格执行安全生产相关管理规定；按照广州市住房和城乡建设委员会的规定发包劳务或使用自有劳务队伍。

八、本公司如投标时选用绿色生产达标企业生产的混凝土，中标后将使用绿色生产达标企业生产的混凝土，若不遵守承诺，自愿按不履行合同约定处理。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
