normal style='text-indent:24.0pt;line-height:120%'>本次招标的是江苏省中医院，占地76亩，建筑面积约20.2万平方米。其中北院占地面积29亩，建筑面积约9.2万平方米，南院占地面积47亩，建筑面积约11万平方米。本次招标分为两个标段，分包一为北院院区（含汉中校区7、8号楼）的保洁、医疗辅助服务。分包二为南院院区保洁、医疗辅助服务、瓦木水电维修，空调维修，小五金维修等物业服务。

normal style='text-indent:24.0pt;line-height:120%'>分包一招标涉及范围：北院保洁（保洁、外勤、运送、陪检、搬家（家俱、加床、其他设备设施的搬移等）、服务调度中心、门帘安装拆卸清洗、窗帘隔帘安装拆卸、控烟、治安协防、消防协防、安全协助管理、资料与信息管理；汉中校区7、8号楼的保洁，病案室病历送取、打包等。

normal style='text-indent:24.0pt;line-height:120%'>分包二招标范围涉及：1、南院保洁（保洁、外勤、运送、搬家（家俱、加床、其他设备设施的搬移等）、24小时服务调度中心、门帘安装拆卸清洗、窗帘隔帘安装拆卸、控烟、治安协防、消防协防、安全协助管理、资料与信息管理。

normal style='text-indent:24.0pt;line-height:120%;text-autospace:none'>2、南院24小时综合运行维修保养（供水供热换热站系统运行；雨水、污水、集水坑、排水系统运行；空调机房、风冷热泵、冷却塔各类风冷热泵、多联机、精密空调、冷却水、冷冻水、送风、排风、新风等空调系统运行；10KV变电室以下配电系统运行；蒸汽系统运行）

normal style='text-indent:24.0pt;line-height:120%'>3、北院24小时综合运行维修保养（空调机房、风冷热泵、冷却塔各类风冷热泵、多联机、精密空调、冷却水、冷冻水、送风、排风、新风等空调系统运行）

normal style='text-indent:24.0pt;line-height:120%'>4、南院24小时综合保养维修：（瓦工、木工、油漆工、五金、铝合金、水、电、机修、防雷与等电位接地）

normal style='text-indent:24.0pt;line-height:120%'>政府采购预算人民币1400万元，服务期为一年。

normal style='text-indent:24.0pt;line-height:120%;text-autospace:none'>投标人可以同时投两个分包，但中标结果只能为一个分包，不能兼得。评标委员会对各分包进行综合评分后确定每个分包的一名中标候选人（若出现一名投标人在两个分包综合评分均为第一的情况，投标人可以选择其中任意一个分包成交，其他分包的中标候选人则为综合评分排名在其后一位的投标人）。

normal style='text-indent:24.1pt;line-height:150%'>三、合格的投标人必须符合下列条件：

normal style='text-indent:24.0pt;line-height:150%'>（一）符合政府采购法第二十二条第一款规定的条件，并提供下列材料；

normal style='text-indent:24.0pt;line-height:150%'>1、法人或者其他组织的营业执照等证明文件，自然人的身份证明；
　　2、2016年度的财务状况报告（成立不满一年不需提供）；

normal style='text-indent:24.0pt;line-height:150%'>3、依法缴纳税收和社会保障资金的相关材料；
　　4、具备履行合同所必需的设备和专业技术能力的证明材料；
　　5、参加政府采购活动前3年内在经营活动中没有重大违法记录的书面声明；

normal style='text-indent:24.0pt;line-height:150%'>（二）本项目不接受联合体投标；

normal style='text-indent:24.0pt;line-height:150%'>（三）未被“信用中国”网站（www.creditchina.gov.cn）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重失信行为记录名单。
