normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">1.投标人应遵循以下程序完成网上投标登记手续：

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">（1）登录广州公共资源交易中心网站投标人服务专区完成投标人的相关信息录入。

normal style="line-height:150%">    （2）核对并确认投标信息无误后，上传带有电子签名及电子签章的加密投标文件。在递交投标文件截止时间前，投标人可以替换投标文件。投标文件须于递交投标文件截止时间前完整上传并保存到广州公共资源交易中心的电子评标系统。如果投标文件于递交投标文件截止时间未能上传完毕，该投标文件将视为无效投标文件。

normal style="text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%">1、投标文件中有法定代表人证明书，如投标文件为委托代理人签署应同时附上法定代表人证明书和法定代表人授权书。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">2、投标人持有建设行政主管部门颁发的企业资质证书及安全生产许可证。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">3、投标人应具备以下资质之一：

（1）投标人具有承接本工程所需的市政公用工程施工总承包三级或以上资质；

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">（2）投标人拟担任本工程项目负责人的人员为：市政公用工程专业二级或以上级别的注册建造师（注：大、中型项目选择此项）；

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》中对应的资质类别及等级的承包工程范围和《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）承接工程。具体原则按《建筑业企业资质管理规定和资质标准实施意见》（建市[2015]20号）执行。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">②建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“取得二级建筑师、二级结构工程师、二级建造师执业资格证书持证人员从业情况信息公开平台”上有效的《二级建造师执业资格证书持证人员从业情况信息公开证明》，《信息公开证明》可不取库，提供纸质文件即可。小型项目负责人需符合粤建市[2010]26号文规定，专业以企业聘书上所聘专业为准，如聘书中专业未明确，以小型负责人培训合格证或小型负责人继续教育培训合格证中的专业为准。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">4、项目负责人持有在有效期内的安全生产考核合格证书（B类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。专职安全人员须具有在有效期内的安全考核合格证书（C类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。项目负责人和安全员不为同一人。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">5、投标人在广州市住房和城乡建设委员会建立了企业诚信档案，拟担任本工程项目负责人为本企业诚信档案中的在册人员。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">6、投标人按规定的格式及内容要求签署了《投标申请人声明》。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">7ink1">、关于联合体投标：ink1">本项目不接受联合体投标。

normal style="margin-left:0pt;text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%;mso-list:l0 level1 lfo1">（2）上述投标人合格条件2至5项（除第5点需提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页外）的信息取自投标人在广州市房建和市政企业库登记的信息，企业库记录的该部分信息将被视为投标申请人递交资格审查资料的一部分。评标委员会对该部分资料的审查将以递交投标文件截止时间在广州市房建和市政企业库记录的信息为依据。投标人应及时维护其在广州市房建和市政企业库登记的信息，确保各项信息在有效期内。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">十四、满足资格审查合格条件的投标人不足3名或通过否决性条款审查的投标人不足3名时为招标失败。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">招标人因两次或多次招标失败，需申请改变招标方式或不招标的，应按《广东省实施〈中华人民共和国招标投标法〉办法》（省第十届人大常委会第二次会议通过2003.4.2）的第四十条规定执行。
