normal style="line-height:150%">四、项目概况：为白江湖森林公园灯芯窿工区，总面积为129.3 公顷（1939.5 亩），实际改造面积为 80.3公顷（1204.5 亩），其中低效桉树林改造 9.2 公顷（138 亩），森林公园林分改造补植套种 71.1公顷（1066.5 亩）。建设以地带性常绿阔叶树为主的碳汇森林，提高森林生态效益。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%">法定代表人证明书原件、法定代表人授权委托证明书原件（非法定代表人参加时提供）、本人身份证原件、《广州建设工程投标报名申请表》；

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:32.0pt;mso-line-height-rule:exaly">2．如出现拟报项目负责人不能被使用而造成投标登记信息无法录入广州公共资源交易中心交易服务系统的，招标人将拒绝接收其投标文件。

normal style="text-indent:26.9pt;mso-char-indent-count:2.24;line-height:32.0pt;mso-line-height-rule:exaly">2、投标人具有广东林学会营造林专业委员会核发的造林工程施工乙级或以上资质（不含公示期内资质，并通过年审）。

3、投标人拟担任本工程项目负责人的人员为林业专业中级或以上技术职称,要求提交项目负责人的职称证书、第二代身份证、本企业聘书。

normal style="text-indent:26.9pt;mso-char-indent-count:2.24;line-height:32.0pt;mso-line-height-rule:exaly">4、投标人自2015年1月1日至今完成过质量合格的类似工程业绩（类似工程业绩是指通过造林工程施工资质完成的单项中标金额大于或等于180万元的造林工程施工业绩）, 需同时提供项目招标公告网页打印件及招标公告网上查询途径（相关网址）、中标公示（告）网页打印件及中标公示（告）网上查询途径（相关网址）、中标通知书、施工合同、竣工验收报告或竣工验收证明、类似工程业绩情况表,业绩时间和造价以中标通知书为准。如以上资料不能证明业绩规模的技术指标（指面积、高度、跨度、管径等）的，须另提供可证明业绩技术指标的其他资料。

注：中标通知书应由以下单位确认：广州公共资源交易中心及其分部、前广东省建设工程交易中心、前广州市建设工程交易中心、前广州市番禺区建设工程交易中心（招标中心）、前广州市花都区建设工程招标管理办公室、前广州经济技术开发区建设工程交易中心、前从化市建设工程交易中心以及前增城市建设工程交易中心。

5、专职安全员须具有安全生产考核合格证（C类），或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的打印页。

normal style="text-indent:24.0pt;mso-char-indent-count:2.0;line-height:32.0pt;mso-line-height-rule:exaly;tab-stops:18.0pt">十四、满足资格审查合格条件的投标人不足3名或经评审有效的投标单位不足3名时为招标失败（当N个标段同时招标且不允许兼中时，满足资格审查合格条件的投标人不足N+2名或经评审有效的投标单位不足N+2名时为招标失败）。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。

normal style="margin-left:0pt;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%;mso-list:l0 level1 lfo1">十五、本工程根据《关于规范房屋建筑工程和市政基础设施工程施工招标控制价设立的通知》（穗建筑[2010]564号）及按广州市地方技术规范《造林建设工程造价编制规范》（DBJ440100/T11-2008)和《广州市营造林工程综合定额（2013）》、《广东省营造林工程综合定额（2010）》、《广州地区有害生物防治工程综合定额（2013）》及相关工程造价编制规范设置招标控制价。
