4、投标人应具备以下资质：

投标人具有承接本工程所需的市政公用工程施工总承包资质壹级或以上级别施工总承包资质；投标人拟担任本工程项目负责人的人员为：市政公用工程专业壹级的注册建造师

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》中对应的资质类别及等级的承包工程范围和《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）承接工程。具体原则按《建筑业企业资质管理规定和资质标准实施意见》（建市[2015]20号）执行。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>②建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>5、项目负责人持有在有效期内的安全生产考核合格证书（B类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。专职安全人员须具有在有效期内的安全考核合格证书（C类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。项目负责人和安全员不为同一人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>6、投标人自2015年1月1日至今完成过质量合格的类似工程业绩（类似工程业绩是指工程造价等于或大于8000万元的市政公用工程施工总承包业绩,完成时间以竣工验收时间为准），需同时提供中标通知书或免招标的相关证明、施工合同、竣工验收报告、工程竣工备案表或建设工程质量监督部门出具的工程竣工合格验收的证明。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：（1）业绩取自广州市房建和市政企业库（未纳入《广州市城乡和建设委员会关于完善企业业绩信息登记的通知》(穗建筑[2016]776号）中“入库范围”的除外）。投标人须提供类似工程业绩网页信息打印页。不提供打印页的业绩不予评审。评标委员会对业绩的评审以投标截止时间在库内业绩上传件为依据。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>本项目不接受联合体投标。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>9、资格审查前，投标人在广州住房和城乡建设委员会建立了企业诚信档案及拟担任本工程项目负责人为本企业诚信档案中的在册人员。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>10、本工程不接受被列入我区不良记录名单（有效期内）的单位或个人（指项目负责人）投标，不良记录具体名单以资格审查时广州市黄埔区、开发区门户网站的广州开发区建设局网页“建筑活动不良行为记录”栏目（网址：http://www.hp.gov.cn/hp/jzhdblxwjl/list.shtml）公布名单（处于通报有效期内）为准。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>11、投标人未在以往工程中因不诚信行为或不充分履约行为被本项目招标人书面拒绝投标的（暂无拒绝投标名单）。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>12、同一标段内的投标人未出现以下情形：与其它投标人的单位负责人为同一人或者存在控股、管理关系的（按投标人提供的《投标申请人声明》第九条内容进行评审）。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>13、具有由公司、企业住所地所属检察-院或广州市黄埔区人民检察-院（对有关单位或个人申请涉及外国国籍人士的个人行贿犯罪档案查询则由广东省人民检察-院受理查询）于该项目招标期间（自招标公告发布之日起到递交投标文件截止期）出具的《检察机关查询行贿犯罪档案结果告知函》，证明投标人及其法定代表人、项目负责人在近10年内(单位营业期限不满10年的即按单位营业执照成立日期起查询)无行贿犯罪记录。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>注：（1）投标人证明符合资审合格条件必须按招标文件投标须知第11.2.2条提供书面资料。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>十一、本工程采用资格后审方式，由评标委员会（技术标评委）负责资格审查，满足资格审查合格条件的投标人不足3名、或通过技术标（或经济标）有效性审查的投标人不足3名时为招标失败。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>十二、招标人因两次或多次招标失败，需申请改变招标方式或不招标的，应按《广东省实施〈中华人民共和国招标投标法〉办法》（省第十届人大常委会第二次会议通过2003.4.2）的第四十条规定执行。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:200%'>十三、资格审查结果及中标结果将在广州公共资源交易中心网站公示，公开接受投标人的监督。

normal align=left style='text-align:left;text-indent:31.45pt;line-height:150%'>四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。
