normal align=left style='text-align:left;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>（1）日常巡查维护：包括根据巡查工作要求，制定巡查计划、路线和方案，安排专人专车对路面的交通护栏等设施进行日常巡查，填报巡查记录表，巡查发现的问题通过拍照、录像或手机等方式记录和报障，实施调直、扶正、加固、补贴反光膜等现场简易修复作业，定期清洗反光镜和安装于石马上的轮廓标，更新路面护栏设施的台账资料等工作。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>投标人拟担任本工程项目负责人的人员为：投标人拟担任本项目负责人为市政公用工程专业贰级或以上注册建造师，项目负责人持有在有效期内的安全生产考核合格证（B类）或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》中对应的资质类别及等级的承包工程范围和《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）承接工程，并按《住房城乡建设部关于简化建筑业企业资质标准部分指标的通知》(建市[2016]226号)执行。

normal style='text-indent:30.0pt;mso-char-indent-count:2.5;line-height:150%'>②建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人应提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“广东省二级注册建造师、二级注册结构工程师、二级注册建筑师注册管理信息系统”上有效的《电子注册证书》，《电子注册证书》可不取库，提供纸质文件即可。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录，本公司没有被人民法院列入失信被执行人名单。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本招标项目监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后不转包或违法分包，在施工过程中，严格执行安全生产相关管理规定；按照广州市住房和城乡建设委员会的规定发包劳务或使用自有劳务队伍。

normal style='text-indent:30.0pt;mso-char-indent-count:2.5;line-height:150%'>八、本公司承诺，中标后将利用信息技术手段，对施工现场人员建立基本信息档案、实行实名管理的制度并完成工资支付，切实落实《广州市住房和城乡建设委员会关于转发<广东省住房和城乡建设厅关于房屋建筑和市政基础设施工程用工实名管理暂行办法>的通知》（穗建筑﹝2018﹞981号）中的各项规定。我公司对实名管理负总责。若本项目实行劳务分包的，我公司对劳务分包企业实施统一管理，监督其用工企业按时足额支付作业工人工资，督促落实实名管理制度。本公司接受招标人及建设行政主管部门的监督、检查。

九、与本公司单位负责人为同一人或者与本公司存在控股、管理关系的其他单位包括：。（注：本条由投标申请人如实填写，如有，应列出全部满足招标公告资质要求的相关单位的名称；如无，则填写“无”。）
