normal style='margin-left:42.25pt;text-indent:-21.25pt;line-height:20.0pt;mso-line-height-rule:exactly;mso-list:l1 level1 lfo3;tab-stops:21.25pt 42.55pt 49.65pt'>1.  由投标申请人代理人持本人身份证携以下资料报名：广州建设工程投标报名申请表；投标申请函（格式自拟）；法定代表人证明书及其身份证；法定代表人授权委托书及代理人身份证（如需委托）；营业执照副本，资质证书副本，安全生产许可证副本，企业技术负责人职称证，拟派项目负责人建造师证，项目技术负责人水利类工程师或以上职称证，造价工程师（2人）注册证、工程师或以上职称证，企业负责人（A证）、项目负责人（B证）及专职安全员（C证）的安全生产考核合格证书，水利五大员（至少各2人）岗位证，标准员证，机械员证，测量员证，试验员证，在“广东省水利建设市场信用信息平台”网已办理信息录入手续截图；信用等级评价截图；投标人曾承接过中小河流域治理工程的企业业绩证明资料[提供中标通知书、合同、网上中标公示查询路径及截图、竣工验收报告（如已竣工）及项目负责人身份证]；2015年至2017年的财务审计报告及审计执业证；投标人2013年以来，未被列入工程建设领域信用体系黑名单、无建设市场主体不良行为记录（包括但不限于围标串标、质量或安全事故、提供虚假材料等，企业自证，代理保留查证权利）；工期及质量承诺书确认回执。以上人员身份证，拟投入本项目人员近三个月在本单位购买的社保凭证（企业注册地社保部门出具，提供网上查询截图和查询路径）。上述资料复印件按顺序A4纸编制目录装订成册加盖公章，一正两副，经查验复印件与原件一致及资料真实后原件退回。

normal style='margin-left:42.25pt;text-indent:-21.25pt;line-height:20.0pt;mso-line-height-rule:exactly;mso-list:l1 level1 lfo3;tab-stops:21.25pt 42.55pt 49.65pt'>2.  为更好了解项目实际情况以利于投标文件编制，于2018年12月29日17:30前组织潜在投标人勘察施工现场，潜在投标人与招标代理预约后由法定代表人、企业技术负责人、拟派项目负责人、项目技术负责人、造价师（2人）、水利五大员（至少各2人）、标准员、机械员、测量员、试验员携投标申请函（格式自拟），营业执照副本，资质证书副本，安全生产许可证副本，企业技术负责人职称证，拟派项目负责人建造师证，项目技术负责人水利类工程师或以上职称证，造价工程师注册证、工程师或以上职称证，企业负责人（A证）、项目负责人（B证）及专职安全员（C证）的安全生产考核合格证书，水利五大员岗位证，标准员证，机械员证，测量员证，试验员证，投标人在“广东省水利建设市场信用信息平台”网已办理信息录入手续截图；信用等级评价截图；投标人曾承接过中小河流域治理工程的企业业绩证明资料[提供中标通知书、合同、网上中标公示查询路径及截图、竣工验收报告（如已竣工）及项目负责人身份证]；2015～2017年经审计财务报表及审计执业证；投标人2013年以来，未被列入工程建设领域信用体系黑名单、无建设市场主体不良行为记录（包括但不限于围标串标、质量或安全事故、提供虚假材料等，企业自证，代理保留查证权利）。以上人员二代身份证，拟投入本项目人员近三个月在本单位购买的社保凭证（企业注册地社保部门出具，提供网上查询截图和查询路径）。上述资料一式两份复印件加盖公章（原件核查后退回）到项目实施地踏勘现场，签订工期及质量承诺书并领取回执，回执作为报名资料组成之一。
