normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>本招标项目 湛江市第二十一中学教学综合楼改扩建工程项目工程已由湛江市霞山区发展和改革局（项目审批、核准或备案机关名称）以湛霞发改字[2017]16号（批文名称及编号）批准建设，项目业主为湛江市第二十一中学，建设资金来自政府投资（资金来源），项目出资比例为 100%  ，招标人为湛江市第二十一中学。项目已具备招标条件，现对该项目的施工进行公开招标。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>2.1、工程名称：湛江市第二十一中学教学综合楼改扩建工程项目工程

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>2.2、建设地点：湛江市霞山区建新东路12号（湛江市第二十一中学校园内）。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>2.3、工程概况：新建一栋六层教学综合楼，建筑面积8300平方米。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>2.4、工程预算总造价：18632821.60元。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>2.5、施工工期：180日历天，工期总日历天数与根据前述计划开竣工日期计算的工期天数不一致的，以工期总日历天数为准。

   2.6、质量要求：施工要求的质量标准：合格。

   2.7、招标范围：工程招标范围按招标文件提供的施工图、工程量清单、有关资料、说明和涉及的标准、规范所涉及的内容，范围包括：施工图、招标文件和有关资料所包含的工程内容。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.1、投标人必须是在中华人民共和国境内注册的独立企业法人或其他组织，能独立承担民事责任，取得合法企业工商营业执照和税务登记、组织机构代码证（若供应商已办理三证合一，则只需提供营业执照）；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.2、投标人必须具有主项或增项建筑工程施工总承包叁级以上(含叁级)及机电工程施工总承包叁级（或以上）或建筑机电安装工程专业承包三级（或以上）资质；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.3、参加本工程项目投标的项目负责人必须是投标单位人员（以其在当地社保局2018年9-11月的社保缴纳证明文件为准），具有建筑工程专业二级（或以上）注册建造师或机电工程专业二级（或以上）注册建造师，(根据粤建市函【2011】218号文，省外注册的二级建造师暂不能跨省在我省注册，故不接受外省企业的二级注册建造师报名), 具有建筑施工企业项目负责人安全生产考核合格证书(B)证或广东省建筑施工企业管理人员安全生产考核信息系统打印的证书；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.4、投标人必须具有有效的建筑施工企业安全生产许可证；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.5、投标人必须提供基本户开户许可证复印件；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.6、投标人在“信用中国”网站（www.creditchina.gov.cn）没有被列入失信被执行人、重大税收违法案件当事人名单（提供公告发布后网站的查询结果打印页面并加盖公章原件）；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.7、根据《广东省住房和城乡建设厅关于取消省外建筑企业和人员进粤信息备案有关工作的通知》（粤建市〔2015〕52号）要求，省外建筑企业需在“进粤企业和人员诚信信息登记平台”录入相关信息并通过数据规范检查，并在投标文件中提供相关信息；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>3.8、本招标工程不接受联合体报名。

说明：1)本次招标实行资格后审，详细资格要求见招标文件。资格审查不合格的投标人投标文件将被否决。2)投标人报名前已在广州公共资源交易中心办理企业信息登记。

报名时提供彩色复印件，所有彩色复印件须加盖投标人公章，并提供原件备查，需要年审的证件必须经有关行政主管部门年审合格才有效，如到期未年审，必须附有关行政主管部门未组织年审的说明，否则视做无效证件：

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.1、投标人的法定代表人证明书原件及身份证复印件、法定代表人授权委托书原件及身份证复印件（法定代表人亲自报名不需要提供授权委托书）；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.2、投标人的营业执照复印件（未改为企业的事业单位，则提供事业单位法人证书；部队单位的，则提供有偿服务许可证）；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.3、投标人的施工资质证书副本复印件；（此项不需要提供原件核对）

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.4、投标人的建筑施工企业安全生产许可证复印件；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.5、有效期内的投标人的基本账户开户许可证复印件。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.6、提供拟派项目负责人身份证复印件、资质证书复印件及其在当地社保局2018年9-11月的社保缴纳证明文件；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.7、提供通过“信用中国”网站（www.creditchina.gov.cn）、中国政府采购网（www.ccgp.gov.cn）等渠道查询的信用信息查询记录网络截图件并加盖投标人公章；

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>4.8、省外企业需提供“进粤企业和人员诚信信息登记平台”该企业和人员信息资料网页的打印件；

注：(1)投标人应按要求提交以上报名资料一份，且投标人应编制报名资料的封面、目录并装订成册；

(2)投标人对以上报名资料的真实性负责，经审查符合要求后方可报名购买招标文件。

(3)报名时投标人的法定代表人或其授权代表必须持本人身份证原件亲自到场报名（注：报名授权代表必须是拟派该项目的项目负责人，办理报名时须携带身份证原件核对），并提供本人身份证复印件及其本单位注册所在地为其购买的2018年9、10、11月份连续时段的社会劳动保险缴费证明（如达到法定退休年龄，须提供行政机关相关证明）

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>5.1、报名时间：2018年12月28日至 2019年 1月2日上午9：30至11：30，下午14：00至16：00。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>5.2、报名地点：广州公共资源交易中心(广州市天河区天润路333号)，招标文件资料费2000元/套,售  

 后不退。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>6.1、凡提供符合要求的报名必须资料的报名人均可成为投标人。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7. 投标文件的递交

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7.1、开标时间：2019年 1月17日 10时 00分。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7.2、开标地点：广州公共资源交易中心05开标室。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7.3、投标文件递交的截止时间(投标截止时间)：2019年1月 17日 10时 00分。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7.4、投标文件递交地址：投标人应于投标截止时间前将投标文件密封送达广州公共资源交易中心第           05号开标室（地址：广州市天河区天润路333号）。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>7.5 、逾期送达的或未送达指定地点的投标文件 ，招标人或招标代理机构不予受理。

normal style='text-indent:21.0pt;mso-char-indent-count:2.0'>本次招标公告同时在广州公共资源交易中心网站、广东省招标投标监管网等媒体上发布。如需对本公告进行修改、补充，将在广州公共资源交易中心网站发布，本公告在各媒体发布的文本如有不同之处，以在广州公共资源交易中心网站发布的文本为准。
