normal style='text-indent:24.0pt;mso-char-indent-count:2.0'>4.2 报名及购买招标文件时须持以下资料：①法定代表人证明书原件和授权委托书原件，法定代表人或授权委托人二代身份证复印件；②营业执照副本复印件； ③最近三个会计年度（2015年～2017年）经合法中介机构审计的财务报表（包括资产负债表、现金流量表、损益表）；④业绩证明材料复印件业绩证明材料复印件（提供中标通知书或合同协议书、完工验收鉴定书和设备运行报告）。投标报名申请表（在广州公共资源交易中心网站下载，并按表格要求填写盖章）一式两份，单独提交。
