5、投标人拟担任本工程项目负责人的人员为：水利水电工程专业二级或以上级别的注册建造师，并持有水行政主管部门颁发的安全生产考核合格证（B类）复印件或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。需提供建造师注册证、有效的第二代身份证、安全生产考核合格证B证、投标截止前近三个月（2018年10月-2018年12月）有效的社保证明资料。

注：建注册证书在有效期内（根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函[2011]218号）的规定，二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业）。投标人提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“取得二级建筑师、二级结构工程师、二级建造师执业资格证书持证人员从业情况信息公开平台”上有效的《二级建造师执业资格证书持证人员从业情况信息公开证明》，《信息公开证明》可不取库，提供纸质文件即可。
