normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>本招标项目雷州市郭宅生活垃圾简易填埋场治理项目已由雷州市发展和改革局以雷发改[2019]9号批准建设，项目发包人为雷州市公用事业局，现采用EPC＋运营服务的模式进行公开招标。

normal style='margin-left:0pt;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%;mso-list:l4 level1 lfo2'>4.  招标控制价：本项目招标分为两部分：设计施工总承包招标控制价为2917.22万元；运营服务费用招标控制价（应急卫生填埋区垃圾填埋服务费：42.2元/吨；应急卫生填埋区渗滤液处理服务费：180元/吨；简易填埋区三年维护服务费：141.27万元），投标报价不得超过招标控制价，否则投标无效。

normal align=left style='margin-left:0pt;text-align:left;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%;mso-pagination:widow-orphan;mso-list:l0 level1 lfo3'>5. 本次招标接受联合体投标，联合体成员不得超过3家。联合体各方必须按规定的格式签订联合体协议，明确联合体牵头方和联合体成员方的主要权利和义务；联合体牵头方可由组成联合体的任意一方担任，但投标人须在联合体协议中明确联合体牵头方，联合体各成员不得再单独或与其他单位组成联合体参与本项目资格预审。

normal align=left style='text-align:left;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%;mso-pagination:widow-orphan'>本项目不组织投标人统一进行现场踏勘。为保障本项目的顺利实施，投标人须充分了解环境的需求并到项目实施地踏勘以充分了解工地位置、情况、道路、储存空间、装卸限制及任何其它足以影响报价的情况，在投标报价中考虑，任何因忽视或误解工地情况而导致的索赔或工期延长申请将不获批准。投标人必须在2019年1月24日前（工作时间，逾期不受理）前往本项目实施地进行踏勘。投标人的施工负责人、设计负责人携带踏勘现场（郭宅生活垃圾简易填埋场）拍摄的同时包括施工负责人、设计负责人的照片（一张）、企业（联合体形式参与的，指联合体各方）出具的授权委托书原件、本人身份证原件、近三个月社保证明原件、施工负责人注册建造师证书原件、营业执照（联合体形式参与的，指联合体牵头方）原件及上述资料的复印件一份(加盖公章)前往广东省国际工程咨询有限公司（详细地址：广州市越秀区环市中路316号金鹰大厦25楼2518室），并取得广东省国际工程咨询有限公司盖章出具的现场踏勘证明，所有委托人员一经确定后不能更换。
