normal style='text-indent:12.0pt;mso-char-indent-count:1.0;line-height:19.0pt;mso-line-height-rule:exactly'>1、主要建设内容：1、第三埇山塘主坝加高培厚，增设溢洪道、重建放水涵；2、第三埇山塘扩容、清淤，建环山集水沟；3、第三埇山塘和大塘水库建设自流钢筋砼压力管汇合后，建设自流钢筋砼压力管引山塘水库水至高桥河边滤水池；4、高桥河边建设渗水渠和集水井2座；5、高桥河边建滤水池2座，建设管理房1处；6、滤水池处建设PE输水管道至原高桥水厂；7、高桥镇原水厂西北面增建反滤、沉淀池1座；8、大禾塘山塘主坝加高培厚，建溢洪道、输水涵；9、大塘水库淤积库容清淤，增加有效库容。

normal style='text-indent:18.0pt;mso-char-indent-count:1.5;line-height:19.0pt;mso-line-height-rule:exactly'>1) 项目负责人 1 名，具有水利水电工程专业贰级（或以上）注册建造师资格（或广东省外注册的水利水电工程专业壹级建造师注册证书），须取得水利行政主管部门颁发的安全生产考核合格证书（B 证），且具备水利类中级或以上职称，并在报名时及中标后，该项目负责人不能担任其它项目的项目负责人；

normal align=left style='text-align:left;text-indent:12.0pt;mso-char-indent-count:1.0;line-height:19.0pt;mso-line-height-rule:exactly'>4)五大员（施工员 1 名、质检员1 名、材料员 1 名、资料员 1 名、安全员 1 名），需具备水利类初级或以上职称和中国水利工程协会颁发水利专业的岗位证，其中安全员须取得水利行政主管部门颁发的 安全生产考核合格证书（水建安 C 证）。

normal align=left style='text-align:left;text-indent:12.0pt;mso-char-indent-count:1.0;line-height:19.0pt;mso-line-height-rule:exactly;layout-grid-mode:char'>（三）由于本项目施工时间紧迫，建设条件复杂，投标人应在投标报价时充分考虑费用。为减少施工期间造成当地乡村道路及环境损坏的影响，投标人应自行标前踏勘以充分了解施工场地的位置、周边环境、道路、装卸、保管、安装限制等情况。投标人的法定代表人、拟派项目负责人、拟派项目技术负责人携带本人身份证原件、注册证(或职称证或资格证或岗位证)原件、社保证明资料原件及上述资料的复印件一份(加盖公章)踏勘现场，原件核查后退回并由招标单位在踏勘证明上盖章(格式见本公告附件一)。踏勘现场时间：踏勘现场时间：2019年1月30日至2019年 2月3日每天上午9:30至11:30，下午14:30至16:00。投标人需提前2小时联系招标代理单位安排踏勘现场。投标人标前踏勘期间，交通工具自备，食宿自理，现场踏勘期间所发生的一切伤亡事故及财产损失由投标人自行承担，招标人不承担任何责任。

 1．企业营业执照副本复印件；

 2．企业资质证书副本复印件；

3．企业安全生产许可证副本复印件；

 4．投标人的基本账户开户许可证复印件（如果基本帐户帐号变更，还必须提供开户行出具的有效帐号变更证明）；

 5．投标人的法定代表人证明书原件、被委托人的有效授权书原件及其被授权人身份证复印件（法定代表人亲自到场报名需提供身份证复印件，无需提供授权委托书）；

 6．法定代表人取得水利行政主管部门颁发的安全生产考核合格证书（A 证）复印件；

7．拟派本项目负责人身份证复印件、资质证书复印件（项目负责人应提交建造师注册证书或建造师临时执业证书）、水利行政主管部门颁发的安全生产考核合格证书（B 证）复印件、职称证复印件及社会劳动保险证明原件；

 8．拟委派本项目项目技术负责人身份证复印件、水利类中级（或以上）职称复印件及社会劳动保险证明原件；

 16．广州建设工程投标报名申请表(原件，一式二份，单独提交)；投标人自行在广州公共资源交易中心网上下载。

 注：报名资料按以上顺序装订成册，并自行增加封面（注明项目名称、投标人名称、单位地址、邮政编码、联系人、联系人移动电话、电子邮箱、报名日期）、目录和页码，一份正本一份副本，所有复印件须加盖投标人单位公章，并提供原件核对。

七、发布公告的媒介

本次招标公告同时在广东省招标投标监管网、广州公共资源交易中心网上发布。如需对本公告进行修改、补充，将在广州公共资源交易中心网站发布，本公告在各媒体发布的文本如有不同之处，以在广州公共资源交易中心网站发布的文本为准。
