受黄河三门峡医院（以下简称“招标人”）,委托中国水利水电第十一工程局有限公司（以下简称“招标机构”）以公开招标方式采购 平板数字胃肠机 壹台/套，采购设备计划使用自有资金用于本次招标后所签订合同的支付。

二、项目概况及招标范围

normal style='line-height:150%'>    2、报价包含：设备的采购、安装、调试、验收、培训、质保期内外服务、与货物有关的运输和保险及其他伴随服务等。具体报价范围、采购范围及所应达到的具体要求，以本招标文件中商务、技术和服务的相应规定为准。

normal style='text-indent:24.0pt;line-height:150%'>3、交货时间：收到中标通知书后30日内，或投标人报出最早交货期。

normal style='text-indent:24.0pt;line-height:150%'>4、交货地点：河南省三门峡市和平路西段黄河三门峡医院。

normal align=left style='text-align:left;line-height:150%'>    5、质量要求：符合该产品国家强制标准、行业标准及卖方企业标准及招标文件要求。

三、投标人资格要求

normal style='text-indent:24.0pt;line-height:150%'>投标人必须满足以下全部资格要求：

normal style='text-indent:24.0pt;line-height:20.0pt'>1、投标人必须是中华人民共和国境内注册成立一年以上，具有独立法人资格，运行状况良好，而且其经营范围包含：医疗器械的生产（制造商）或销售（代理商）。

normal style='text-indent:24.0pt;line-height:150%'>2. 投标人须为专业从事医疗设备的销售企业，符合《医疗器械监督管理条例》的要求，公司应提供营业执照、经营许可证及产品注册证/表、生产许可证、授权书等合法有效证件，并提供相应供货合同书复印件,资质证件不全的按废标处理。

normal style='text-indent:24.0pt;line-height:150%'>3.具有履行本项目所必需的设备和专业技术能力。

normal style='text-indent:24.0pt;line-height:150%'>4.投标人财务状况良好，近三年没有处于被责令停业，财产被接管、冻结、破产状态。

normal style='text-indent:24.0pt;line-height:150%'>5.投标人是增值税一般纳税人，能开具增值税发票。

normal style='text-indent:24.0pt;line-height:150%'>6.本次招标不接受联合体投标。

normal style='text-indent:24.0pt;line-height:150%'>7.投标人不存在《中华人民共和国招标投标法》、《中华人民共和国招标投标法实施条例》和《政府采购法》禁止投标的情形。

1、凡满足本公告规定的投标人资格要求并有意参加投标者，2019年3月19日下午17:00（北京时间，下同）前在中国电建集中采购电子平台（http://ec3.powerchina.cn）注册、在线报名并上传下列资料（合并文件上传）：购买招标文件经办人身份证和法定代表人签发的针对本招标项目购买招标文件授权委托书或介绍信（加盖公章）扫描件。

normal style='text-indent:24.0pt;line-height:150%'>2、在报名通过后再支付标书费，支付标书费后请在上述时间内在中国电建集中采购电子平台（http://ec3.powerchina.cn）下载招标文件。

normal style='text-indent:24.0pt;line-height:150%'>3、招标文件每套工本费为人民币（大写）：陆佰元整（￥600元），支付招标文件费采用现金方式支付，在开标现场交纳，售后不退。(为方便投标人参与投标，标书费统一在开标现场缴纳现金；在线报名时支付标书费用方式选择上传“缴费凭证”的方式，由投标人制作相关说明内容上传至“缴费凭证”位置，内容可为“招标人要求现场缴纳标书费用，故不再上传具体缴费凭证”,或将此页面截图上传。)

normal style='margin-left:-4.1pt;text-indent:2.15pt;line-height: 150%'>     1、投标文件递交的截止时间（投标截止时间）为2019年4月9日上午10时00分，投标文件递交地点为河南省郑州市高新区莲花街59号中电建十一局715会议室

normal style='text-indent:24.0pt;line-height:150%'>2、逾期送达的或者未送达指定地点的投标文件，招标人不予受理。

normal style='text-indent:24.0pt;line-height:150%'>3、投标截止时间及递交地点如有变动，招标人将及时以书面形式通知所有已确认报名并下载招标文件的潜在投标人。
