四、项目概况：项目实施道路及堤岸工程，包含东门桥与沿江西路交叉口建设（含简单立体交叉工程）；沿江西路改造段长250米，下穿东门桥；中山路改造段长80米，与东门桥相接。道路等级均为城市支路，双向二车道，沿江西路设计速度为20km/h，中山路设计速度为30km/h，改造宽度按照现状道路宽度。主要建设内容包括道路、桥涵、堤岸、交通设施及交通疏解、排水、照明、绿化景观和东门桥车行桥面铺装等工程（招标人有权按实际设计情况微调工程建设规模或建设标准，工程具体实施规模及建设内容最终以经职能部门审定的施工图和招标人确认为准。）

五、标段划分及各标段招标内容、规模和招标控制价：

normal style='text-indent:25.1pt;mso-char-indent-count:2.24;line-height:200%'>（1）投标人具有承接本工程所需的①ink13'>ink14'>工程勘察综合类资质，或工程勘察（岩土工程（勘察）和工程测量）专业类乙级或以上资质ink14'>；②ink14'>工程设计综合类甲级（或以上），或市政行业乙级（或以上）设计资质，或市政行业（道路工程、排水工程）专业乙级（或以上）设计资质；③市政公用工程施工总承包贰级（或以上）资质。投标人必须同时具有①、②、③项资质；

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>施工业绩要求：自2016年1月1日至今完成过质量合格的类似工程业绩（类似工程是指3000万或以上的市政公用工程）。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>注： 业绩取自广州市房建和市政企业库。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>(1)、金额以中标通知书为准，中标通知书上没有金额或免招标的，以施工合同（不含补充合同）为准。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>(2)、证明业绩规模的技术指标需在广州市房建和市政企业库内业绩上传件中反映。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>(4)、 库内业绩上传件不符合上述要求的或登记的工程资质内容与施工合同等业绩证明资料不相符的，该项业绩不予认定。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:200%'>9.1允许联合体投标，但只接受最多由3家单位（其中一家勘察单位、一家设计单位、一家施工单位）组成的联合体，应以承担施工任务的一方为主办方，并签定联合体共同投标协议书。联合体共同投标协议书应明确约定各方拟承担的工作和责任。联合体各方共同与发包单位签订总承包合同，就约定事项承担连带责任。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录，本公司没有被人民法院列入失信被执行人名单。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的招标文件编写、监理工作；本公司与承担本招标项目的代建单位、项目管理单位、造价咨询单位、招标代理单位和监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的工程总承包项目负责人、施工项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后不转包或违法分包，在施工过程中，严格执行安全生产相关管理规定；按照广州市住房和城乡建设委员会的规定发包劳务或使用自有劳务队伍。

八、本公司如投标时承诺选用绿色生产达标企业生产的混凝土，中标后将使用绿色生产达标企业生产的混凝土，若不遵守承诺，自愿按不履行合同约定处理。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
