normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>3、招标内容：采用设计施工总承包形式，工程内容包括拆除原建筑装修；对公共区域装修、给排水、电气、空调通风、通讯等设计施工，特别是四楼需要考虑餐饮业排烟、给排水等要求；对外环境及立面修复；对停车场环境提升等。按照合同约定对工程建设项目的设计、采购、施工、试运行全过程承包。采用综合单价包干形式，承包单位对其项目的质量、安全、费用和进度进行负责，包括但不限于以下内容：

4、投标人同时具有承接本项目所需的4.1、4.2资质：

4.1、工程设计资质： 工程设计综合甲级资质，或建筑行业设计乙级或以上资质，或建筑行业（建筑工程）设计乙级或以上资质，或建筑设计事务所资质，或建筑装饰工程设计专项乙级或以上资质，或建筑装饰装修工程设计与施工一体化贰级或以上资质（须在有效期内）；

4.2、施工资质：具有承接本工程所需的建筑装修装饰工程专业施工贰级或以上资质，或建筑装饰装修工程设计与施工一体化贰级或以上资质（须在有效期内）；

注：资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》中对应的资质类别及等级的承包工程范围和《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）承接工程，并按《住房城乡建设部关于简化建筑业企业资质标准部分指标的通知》(建市[2016]226号)执行。

5、主要负责人资格要求：

5.1投标人拟担任本工程项目负责人（兼施工项目负责人）为： 建筑工程 专业贰级或以上注册建造师资格。

normal style='text-indent:26.9pt;line-height:22.0pt;mso-line-height-rule:exactly;background:white'>注：建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人应提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“广东省二级注册建造师、二级注册结构工程师、二级注册建筑师注册管理信息系统”上有效的电子注册证书。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。项目负责人和安全员不为同一人。

5.2投标人拟担任本项目设计负责人须具备本专业（含相近专业）高级工程师或从事本专业工作10年以上的工程师职称，设计负责人与项目负责人不能为同一人。

6、施工项目负责人持有项目负责人在有效期内的安全生产考核合格证书（B类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。

normal align=left style='text-align:left;text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%;mso-pagination:widow-orphan;tab-stops:5.25pt;layout-grid-mode:char'>2、施工费用由投标人根据招标文件要求以及企业自身情况填报下浮率，投标报价不得高于施工费的最高投标限价。投标人根据招标文件要求以及自行设计施工图编制投标报价。中标后，中标单位根据经审定的方案进行施工图设计、深化设计，并按国家、广东省、广州市以及现行有关工程造价管理的文件分别编制施工图预算，施工图预算必须预留不少于分部分项工程费的10%的费用作为暂列金额，最后报送招标人委托的第三方评审机构评审，招标人结合第三方评审机构评审结果确定施工图预算，经审定的施工图预算按中标下浮率（下浮率为中标价相对最高投标限价的下浮率，下浮率=（最高投标限价-中标价)/ 最高投标限价下浮后的工程造价作为合同价（含暂列金额）。本项目施工费用实行施工图及其预算总价包干。对于法律法规变化、招标人（即发包人）提出的工程变更和提前竣工，不可抗力增加的价款可以从暂列金额中支出，超出暂列金额部分可以调整合同价款，最终结价不得超出批复概算的工程费用。
