本项目投标担保 4 万元，投标担保方式请参照招标文件要求，投标担保须在开标前完成缴纳。

十、资格审查方式：

本项目采用资格后审方式。

十一、投标人合格条件：

1.投标人参加投标的意思表达清楚，投标人代表被授权有效。

2.投标人必须是在中华人民共和国注册的独立法人。投标人持有有效的工商行政管理部门核发的法人营业执照或各级政府事业单位登记管理机关颁发的事业单位法人证书，按国家法律经营。单位负责人为同一人或者存在控股、管理关系的不同单位，不得参加同一标段投标或者未划分标段的同一招标项目报名。

注：1.同类项目是指轨道交通供电系统直流馈线保护装置改造项目，需提供合同、经业主确认的验收报告（或用户评价证明）。

2.单独招标的项目业绩，需同时提供中标通知书、施工合同、竣工验收报告或竣工验收证明。由总承包单位依法分包的项目业绩，只需提供经业主和总承包单位确认的分包合同、竣工验收报告或竣工验收证明。

投标人已按照附件一的内容签署盖章的投标申请人声明。

关于联合体投标：

本项目不允许联合体投标。

在本公告发布时，投标人未在以往广州地铁工程项目中因违约被本业主书面拒绝投标（期限内）（名单详见附件三）。

投标人提交《材料、设备及工器具质量保证和合法供货渠道保证承诺函》

8.

未在招标公告第十一点单列的资审合格条件，不作为资审不合格的依据。

十二、招标公告网上发布时，同时发布招标文件、招标控制价。

发售招标文件结束之日起开始计算编制投标文件时间，编制投标文件的时间不得少于20天。

二、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录。

四、严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本招标项目监理业务的单位没有隶属关系或其他利害关系。

六、如我司成为本项目中标候选人，我司同意并授权招标人将我司投标文件商务部分的人员、业绩、奖项等资料进行公示。（房屋建筑和市政基础设施工程建设项目）
