注：1、“主要人员”指实际参加本项目施工的项目管理、技术等方面的负责人，包括但不限于项目负责人、技术负责人、材料工程师、质量负责人、安全负责人、专职安全员、安全员、资料员、安装工程师、装修工程师、施工员及其他招标人要求配置的人员等。

2、其它人员由投标人视本工程具体情况结合招标人对本工程的具体要求自行配备。

3、投入主要人员需根据《项目管理部主要人员配备基本要求表》要求在表后附相关证明材料复印件。

5、各岗位人员不得相互兼职。

6、项目负责人需提供近半年三甲医院体检报告复印件，加盖公章。

7、我单位承诺上表拟委派人员需在中标后派驻现场办公，接受业主和监理的考勤及监管，如未按承诺执行，同意业主根据合同条款进行扣款处理。
