1、 （以下简称“招标机构”）受招标人委托，邀请合格投标人就深圳市宝安区中心医院移动式双向荧光成像系统采购的下列货物和有关服务提交密封投标：

2、有意向的合格投标人可在 得到进一步的信息和查阅招标文件。
3、资格证明文件和业绩：
（1）投标人必须是来自中华人民共和国或是与中华人民共和国有正常贸易往来的国家或地区的法人或其他组织，并在 注册成会员；须提供营业执照或企业注册证明复印件。
（2）国内投标人须为深圳市政府采购中心注册的供应商；（由招标机构在深圳市政府采购中心系统中查询，投标人无需提供证明材料）
（3）投标人必须是所投产品（移动式双向荧光成像系统）的制造商或制造商有效授权的合法代理商，并提供设备制造商专项授权书或代理证书复印件（投标人是设备制造商的除外），并加盖投标人公章。一家制造商只能授权一家投标人投标。
（4）国内投标人需具有《中华人民共和国医疗器械生产许可证》或《中华人民共和国医疗器械经营企业许可证》（提供证书复印件并加盖投标法人公章）。
（5）投标人所投设备（移动式双向荧光成像系统）必须具有《中华人民共和国医疗器械注册（备案）证》（注册证须含附件：医疗器械产品生产制造认可表或医疗器械产品注册登记表）（提供证书复印件并加盖投标法人公章）（开标时，该证应在有效期内；若不在有效期内，则需提供该证和所投产品在该证有效期内生产的药监局出具的证明文件）。
（6）参与本项目政府采购活动前三年内，在经营活动中没有重大违法记录（必须按照招标文件格式IV－11. 声明函格式内容提供书面声明函）；
（7）参与本项目政府采购活动时不存在被有关部门禁止参与政府采购活动且在有效期内的情况；（必须按照招标文件格式IV－11. 声明函格式内容提供书面声明函）
（8）本项目不接受联合体投标，不允许分包。

4、招标文件获取：

（2）本项目免招标文件订购费，用户仅需购买电子招投标服务即可参与项目。

1. Shenzhen International Tendering Co.,Ltd. (hereafter called “Tendering Agent”)， entrusted by the purchaser, invites sealed bids from eligible suppliers for the supply of the following goods and/or service of Mobile bidirectional fluorescence imaging system procurement project.

2. Eligible bidders can obtain further information and the bidding documents from Shenzhen International Tendering Co.,Ltd.
3. Qualification and Performance Requirements For Bidder:
(1) Bidders must be legal persons or other organizations, which come from People's Republic of China, countries or regions where there are regular trades with People's Republic of China, with legal and financial independence and legal operation, separating from tenderee and tendering agent, and must register as a member on the China international bidding website(( ). Bidders should provide a copy of business license or enterprise registration certification.
(2) Domestic bidders must register on Shenzhen government procurement center. (The tendering agent will inquire in the system of Shenzhen government procurement center. Bidders do not need to provide materials.)
(3) Bidders must be the manufacturers or legal agents which hA/Ve been authorized by the manufacturers, and must provide the copy of the letter of authorization of the equipments from the manufacturers or the copy of the proxy certificate. (Except for bidders who are the manufacturers of the equipments.) One manufacturer can only authorize one bidder.
(4) Domestic bidders must hA/Ve “Production Enterpriser License of Medical Instrument of the People's Republic of China” or “Business License of Medical Instrument of the People's Republic of China”. (Provide the copy of license with official seal of bidders.)
(5) The Mobile bidirectional fluorescence imaging system must hA/Ve “Registration Certificate For Medical Device of the People's Republic of China”. (Provide the copy of license with official seal of bidders.)
(6) During the three years before participating in the government procurement activities of this project, there were no major illegal records in the business activities.
(7) No prohibition by relevant departments from participating in the government procurement activities and within the validity period when participating in the government procurement activities of this project。
(8) This project does not accept joint bidding, and does not allow subcontracting.
4. Acquisition of Bidding documents

(2)This project sells for free. Bidders can participate in the project after purchasing the online bidding service.
(3)The charge of the online bidding service: RMB1000. The platform will issue electronic invoice after payment. If bidders need paper invoice, the invoice will be delivered within a week after payment. The charge of delivery will be paid by bidders.
