normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>8、《查询行贿犯罪档案结果告知函》（参加本项目的投标人（若为联合体投标的各成员单位均须提供）须向本企业注册地或是业务发生地人民检察-院申请行贿犯罪档案查询，并取得《查询行贿犯罪档案结果告知函》。未进行行贿犯罪档案查询或经查询曾有行贿犯罪行为的相关单位不得参与本项目投标。（查询对象包括投标企业、企业法人代表；查询内容为查询对象在本项目招标公告发布之日前3年内有无行贿犯罪记录；查询时间要求在截至本公告发布之日前近3年，企业成立不足3年的，按成立时限提供）。）

normal style='text-indent:21.0pt;line-height:150%'>1．报名时间： 2017年6月 16 日至6月22日（上午9：30～11：30，下午13：30～16：00）。

normal style='text-indent:21.0pt;line-height:150%'>注：报名时间自招标公告截止发布之日起不得少于5日。如报名参加投标的申请人数量过少不足以形成充分竞争时，招标人在确认正式投标人之前，可以发出补充公告，适当延长报名时间。

normal align=left style='text-align:left;text-indent:21.1pt;line-height:150%;mso-pagination:widow-orphan'>十一、本项目评标办法采用综合评估法，投标人的诚信评价分应当采用发布招标公告第1天（备注：本项目招标公告发布第一天视同为投标报名第1天），诚信评价信息系统中发布的最新一期的分数。诚信评价分数在广州市水务工程企业信息库及诚信中心网（http://121.8.226.19/gzswcx/index.jsp）中的“诚信排名>>勘察—给排水”或“诚信排名>>设计—给排水”中“诚信综合评价”中查询。未入库企业或在广州市水务工程企业信息库及诚信中心网中尚无诚信排名的投标人，其诚信评价分按基准分计算，计算方法如下：市场行为基准分为70分，质量管理评价及履约评价基准分均为60分，则诚信评价分=70*0.2+60*0.6+60*0.2=62分。以联合体投标的，根据联合体各方承担的工作内容，提取勘察和设计诚信评价分。
