normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》、《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）以及《住房城乡建设部关于简化建筑业企业资质标准部分指标的通知》(建市[2016]226号)执行。

normal style='text-indent:23.6pt;mso-char-indent-count:2.0;line-height:150%'>②招标内容含有设计要求，且设计要求仅为深化设计的，在投标人的资质设置要求中，不允许设置设计资质要求。建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“取得二级建筑师、二级结构工程师、二级建造师执业资格证书持证人员从业情况信息公开平台”上有效的《二级建造师执业资格证书持证人员从业情况信息公开证明》，《信息公开证明》可不取库，提供纸质文件即可。小型项目负责人需符合粤建市[2010]26号文规定，专业以企业聘书上所聘专业为准，如聘书中专业未明确，以小型负责人继续教育培训合格证中的专业为准。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

6、专职安全人员须具有在有效期内的安全生产考核合格证书（C类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。

7、投标人自2013年 1月 1日至今完成过质量合格的类似工程业绩（类似工程是指中标金额大于或等于9000万元的市政公用工程，完成时间以竣工验收时间为准，中标金额以中标通知书为准）。需同时提供中标通知书或免招标的相关证明、施工合同、竣工验收报告或竣工验收证明复印件。如以上资料不能证明业绩规模的技术指标（指面积、高度、跨度、管径等）的，须另提供可证明业绩技术指标的其他资料（招标人在中标结果公示时将进行公示）。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>注：（1）单独招标的工程业绩，需同时提供中标通知书或免招标的相关证明、施工合同、竣工验收报告或竣工验收证明。由总承包单位依法分包的专业工程业绩，需同时提供总承包单位的中标通知书或免招标的相关证明（复印件）、经业主和总承包单位确认的分包合同、竣工验收报告或竣工验收证明。如以上资料不能证明业绩规模的技术指标（指面积、高度、跨度、管径等）的，须另提供可证明业绩技术指标的其他资料。

normal style='text-indent:48.0pt;mso-char-indent-count:4.0;line-height:200%'>（2）招标人可以要求投标人具备完成过质量合格的类似工程业绩，并可对工程业绩设置一个指标要求，或设置多个指标要求但仅需满足其中之一即可。指标仅限于工程规模、类似的特殊结构形式、类似的特殊施工工艺等方面，有关造价、面积、长度等规模性量化指标要求不得超过招标工程本身所对应的指标的三分之二。对于所需企业资质级别已是最低资质等级的，不得设置业绩要求。

9、关于联合体投标：本项目不接受联合体投标。

十一、招标人因两次或多次招标失败，需申请改变招标方式或不招标的，应按《广东省实施〈中华人民共和国招标投标法〉办法》（省第十届人大常委会第二次会议通过2003.4.2）的第四十条规定执行。

本公司就参加                            投标工作，作出郑重声明：

一、本公司保证本次投标活动所提供的材料都是真实的；在广州公共资源交易中心企业库登记的一切信息都是真实的、在有效期内的。

二、资格审查前，投标人须在广州市住房和城乡建设委员会建立企业诚信档案及拟担任本工程项目负责人须是本企业诚信档案中的在册人员。

三、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

四、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标资格审查截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标资格审查截止时间内被人民法院判决犯有行贿罪的记录。

五、严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定。选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

六、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

七、本公司保证本项目负责人本人凭身份证按招标文件规定亲自签到。

八、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

九、本公司承诺：中标后按照《广州市建筑工程劳务分包管理办法（试行）》（穗建筑〔2013〕428号）的规定发包劳务或使用自有劳务队伍。

十、本公司保证本项目投标文件编制、解释负责人按招标文件规定完成评标委员会提出的澄清要求（如有）。
