注：1、具有相应的建筑装饰装修工程设计与施工一体化一级资质（以资质证书许可范围为准，在有效期内）的单位允许同时承担设计和施工任务。

2、根据《住房和城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）要求，nth="7" Year="2016" w:st="on">2016年7月1日起，按照建建[2001]82号文标准取得的旧版建筑业企业资质证书失效。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：（1）建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>允许联合体投标，则联合体成员不超过两方（一方为设计单位、一方为施工单位），且联合体应以具有本公告5.2资质条件的施工单位为主办方，签订联合体投标协议。投标人拟任本工程项目负责人应为主办方正式员工。联合体工作协议应明确确定联合体主办方以及约定各方拟承担的工作和责任。

广州市荔湾区建设工程招标管理办公室、广州市南海中学：

本公司就参加                       投标工作，作出郑重声明：

一、本公司保证投标报名材料及其后提供的一切材料都是真实的。

二、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员。本公司严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的前期工作、招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后按照《广州市建筑工程劳务分包管理办法（试行）》（穗建筑﹝2013﹞428号）的规定发包劳务或使用自有劳务队伍。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
