normal style='text-indent:23.25pt;line-height:22.0pt;mso-line-height-rule:exaly'>本梅州市梅县区中医医院改扩建工程施工已由梅县发展和改革局以“梅县发改〔2013〕28号文批准建设，招标人为梅州市梅县区中医医院，建设资金来自除申请中央专项资金补助外，其余不足部分由地方政府统筹解决。项目已具备招标条件，现对该项目的工程施工进行公开招标。

normal style='text-indent:23.25pt;line-height:22.0pt;mso-line-height-rule:exaly'>工程概况：项目总建设面积11366平方米，主要包括：改、扩建门诊部、住院病房、中医康复医学科及其它附属工程。扩建配电房，献血大厅，中医楼及门诊楼土建装饰、动力、给排水、消防控制室、火灾自动报警、通风、地下消防水池及水泵房等。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:22.0pt;mso-line-height-rule:exaly;layout-grid-mode:char'>3.1本次招标要求投标人须同时具备房屋建筑工程施工总承包三级（含三级）或以上资质及消防设施工程专业承包资质二级或以上资质的独立法人，持有效的企业营业执照和安全生产许可证，并在人员、设备、资金等方面具有相应的施工能力。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:22.0pt;mso-line-height-rule:exaly;layout-grid-mode:char;background:white'>3.4、财务状况资料。需提供近三年（2014年~2016年）的经会计师事务所出具的完整审计报告，包括资产负债表、现金流量表、利润表等的复印件。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:22.0pt;mso-line-height-rule:exaly;layout-grid-mode:char;background:white'>3.5、现场踏勘证明。由于该项目属于医院改扩建工程，施工现场管线比较复杂，施工过程中要考虑医院的特殊性经营情况，施工难度比较大，为保证该项工程的施工顺利安全进行，要求潜在投标人在报名前，应先跟招标代理取得联系并确认后，携带其本人二代身份证复印件及资格证原件、法定代表人证明书原件，先到招标代理机构处核实身份证原件。由投标人拟派项目负责人、注册造价工程师到现场进行踏勘（现场勘查证明格式自拟）。

normal style='line-height:22.0pt;mso-line-height-rule:exaly'>    3.7、本项目不允许联合体投标。投标人还应在广州公共资源交易中心已办理投标企业IC卡，无IC卡号的资格申请将不予受理。IC卡的办理详情参见广州公共资源交易中心网站服务指南栏目。

normal style='line-height:22.0pt;mso-line-height-rule:exaly'>    3.8、投标人已按照《梅州市建筑市场诚信管理办法（修订）》的要求和梅州市住房和城乡建设局《关于建设工程企业诚信信息登记有关问题的通知》（梅市建函〔2016〕365号）的要求在梅州市住建局建筑市场诚信管理平台自行申办企业信息登记手续并已在该平台公开发布。
