4、投标人具有承接本工程所需的市政公用工程施工总承包资质二级或以上级别施工总承包资质；

投标人拟担任本工程项目负责人的人员为：市政公用工程专业贰级或以上级别注册建造师；

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>②建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。
    根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业投标人应提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“广东省二级注册建造师、二级注册结构工程师、二级注册建筑师注册管理信息系统”上有效的在电子注册证书。    项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

5、持有项目负责人安全培训考核合格证明或在有效期内的安全考核合格证书（B类）的上传件，或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的打印页。

6、专职安全人员须具有安全生产考核合格证明或在有效期内的安全考核合格证书（C类）的上传件，或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的打印页，项目负责人和安全员不为同一人。

7、投标人自nth="1" Day="1" IsLunarDate="False" IsROCDate="False" w:st="on">2015年1月1日至今完成过质量合格的类似工程业绩（类似工程业绩是指中标金额大于或等于900万元的市政公用工程施工总承包工程）。

注：①业绩取自广州市房建和市政企业库。投标人须提供类似工程业绩网页信息打印页，不提供打印页的业绩不予评审。评标委员会对业绩的评审以投标截止时间在库内业绩上传件为依据。

②金额以中标通知书为准，中标通知书上没有金额或免招标的，以施工合同（不含补充合同）为准。

③完成时间以竣工验收报告为准。

④库内业绩上传件不符合上述要求的或登记的工程资质内容与施工合同等业绩证明资料不相符的，该项业绩不予认定。

8、投标人已按照附件一的内容签署盖章的投标申请人声明。

十一、资格审查方式：
