normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>1、投标人参加投标的意思表达清楚，投标人代表被授权有效。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>2、投标人均具有独立法人资格，按国家法律经营。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>3、投标人均持有建设行政主管部门颁发的企业资质证书及安全生产许可证。

4、投标人应具备以下资质：投标人具有承接本工程所需的市政公用工程施工总承包资质壹级或以上级别施工总承包资质；

投标人拟担任本工程项目负责人的人员为：市政公用工程专业壹级注册建造师；

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》中对应的资质类别及等级的承包工程范围和《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）承接工程以及《住房城乡建设部关于简化建筑业企业资质标准部分指标的通知》(建市[2016]226号)执行。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>②建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

5、项目负责人具有在有效期内的安全生产考核合格证书（B类），或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的打印页；

6、专职安全人员须具有在有效期内的安全生产考核合格证书（C类），或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的打印页；项目负责人和安全员不为同一人。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>7、投标人自nth="1" Year="2015" w:st="on">2015年 1月 1日至今完成过质量合格的类似工程业绩（类似工程是指中标金额大于或等于6500万元的市政公用工程施工总承包业绩）。

normal style='text-indent:24.1pt;mso-char-indent-count:2.0;line-height:150%'>注：①业绩取自广州市房建和市政企业库。投标人须提供类似工程业绩网页信息打印页，不提供打印页的业绩不予评审。评标委员会对业绩的评审以投标截止时间在库内业绩上传件为依据。)

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>②完成时间以竣工验收报告为准。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>③库内业绩上传件不符合上述要求的或登记的工程资质内容与施工合同等业绩证明资料不相符的，该项业绩不予认定。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>④金额以中标通知书为准，中标通知书上没有金额或免招标的，以施工合同（不含补充合同）为准。

8、投标人已按照附件一的内容签署盖章的投标申请人声明。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:145%;layout-grid-mode:char;background:white'>9、关于联合体投标：本项目不接受联合体投标。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>10、资格审查前，投标人须在广州市住房和城乡建设委员会建立企业诚信档案及拟担任本工程项目负责人须是本企业中的在册人员。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>11、投标人未在以往工程中因不诚信行为或不充分履约行为被本项目招标人书面拒绝投标；未被纳入联合惩戒范围（穗铁企【2017】51号）。（详见附件二）

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>12、投标人未出现以下情形：与其它投标人的单位负责人为同一人或者存在控股、管理关系的（按投标人提供的《投标申请人声明》第九条内容进行评审）。

注：未在招标公告第十条单列的资审合格条件，不作为资审不合格的依据。

十一、资格审查方式：

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>本工程采用资格后审方式，由评标委员会负责资格审查。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%;layout-grid-mode:char;mso-layout-grid-align:none'>十二、满足资格审查合格条件或通过有效性审查的投标人不足3名时为招标失败。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。
