3. 投标人资格：

(3)投标人必须是所投产品的制造商或制造商有效授权的合法代理商，并提供设备制造商专项授权书或代理证书复印件（投标人是设备制造商的除外），并加盖投标人公章，且在招标机构登记并购买了招标文件。
(4)投标人不得与买方或招标机构有利害关系。
(5)两家以上投标人的投标产品为同一家制造商或集成商生产的，按一家投标人计算。
(6)国内投标人需具有《中华人民共和国医疗器械生产许可证》或《中华人民共和国医疗器械经营企业许可证》（提供证书复印件并加盖投标法人公章）。
(7)投标人所投设备必须具有《中华人民共和国医疗器械注册证》（提供证书复印件并加盖投标人公章）。
(8)法定代表人证明书或法定代表人授权书。
4.有意向的投标人可从即日起至2019年2月1日17时00分止(节假日除外)，每天上午9：00～11：30、下午2：30～5：00 (北京时间) 在惠州市花边北路3号宏城商务大厦5层506室 ( 惠州经营部)现场购买招标文件（请购买人携带提供营业执照副本复印件加盖公司公章或商务执业许可证复印件加盖公司（原件备查）、法定代表人证明书原件、法人代表授权书（购买招标文件）原件、提供所投产品中华人民共和国医疗器械注册复印件加盖公章），本招标文件每包售价人民币300元或45美元，售后不退。
