normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>(1)完成本项目的勘察设计工作（包括但不限于投标阶段的方案设计及初步设计，中标后进行工程勘察（工程测量）、工程方案及初步设计深化、前期摸查、设计概算、施工图设计、施工图预算、竣工图编制等后续所有勘察设计和现场服务工作）；完成本项目施工工作：施工至工程竣工验收、工程结算、工程保修以及项目移交等（包括但不限于沥青路面刨铺、人行道透水砖更换、排水设施改造（含井盖）、绿化升级改造、桥梁清洗及涂装、局部路段建筑立面整饰、招牌整治、特色路面或地面浮雕、特色标识、栏杆改造、公共艺术小品、城市家具更新、交通标识标线更新等内容）。完成并配合业主办理报建、报批、相关部门结（决）算审计等工作。

4.招标规模：越秀区新河浦片区环境改造提升工程占地面积约144公顷，拟改造的道路长约4.4km，道路红线宽度约7-26米，道路改造面积约50453平方米。道路等级为城市次干路、城市支路。本项目总投资约4439.47万元，工程费约3552.63万元，其中市政综合工程费约2550万元，建筑整饰工程费约964万元。拟进行建筑立面整饰的住宅层高8~9层。（最终的规模以相关部门批复为准）

normal style='line-height:150%'>     5、本项目总投资约4439.47万元，本次招标最高限价为3789.41万元。其中：（1）施工投标最高限价为人民币3552.63万元；（2）勘察设计费投标总价最高限价为236.78万元（其中勘察费39.08万元；设计费197.70万元）。

5、投标人同时具有承接本工程所需的5.1、5.2、5.3资质：

5.1工程勘察资质：工程勘察综合甲级资质，或工程勘察专业（工程测量）丙级或以上资质。

5.2工程设计资质：工程设计综合类资质甲级，或同时具有①建筑工程设计行业丙级资质或以上、或建筑行业（建筑工程）专业设计丙级或以上、或建筑装饰工程设计专项乙级或以上资质、或建筑工程设计事务所资质；②市政公用工程设计行业乙级或以上资质、或具备市政行业（道路工程）专业乙级或以上资质。

5.3工程施工资质：具有市政公用工程施工总承包贰级或以上资质。

注：①资质标准按照建市[2014]159号文颁布的新版《建筑业企业资质标准》、《住房城乡建设部关于建筑业企业资质管理有关问题的通知》（建市[2015]154号）以及《住房城乡建设部关于简化建筑业企业资质标准部分指标的通知》(建市[2016]226号)执行。

②经招标人同意，中标人可以把中标项目的部分非主体、非关键性工作分包给具有相应资质和能力的专业单位实施，但不得再次分包。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>8、投标人（或联合体主办方）专职安全员须具有在有效期内安全生产考核合格证（C证），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。项目负责人和安全员不为同一人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：1、参加联合体的各成员不得再以自己的名义单独投标，也不得同时参加两个或两个以上的联合体投标。出现上述情况者，其投标和与此有关的联合体的投标将被拒绝。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>1、本项目采用资格后审方式（投标人资格审查与评标同时进行，资格审查及评标由本项目的评标委员会负责）。

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>2、满足资格审查合格条件的投标申请人不足3名或经评审同时通过设计部分和施工部分有效性审查的投标人少于3人时，均为招标失败。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。

normal align=center style='text-align:center'>投标申请人声明

广州市住房和城乡建设委员会、本招标人及招标监管机构：

本公司就参加                       投标工作，作出郑重声明：

一、本公司保证投标报名材料及其后提供的一切材料都是真实的。

二、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录，本公司没有被人民法院列入失信被执行人名单。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后不转包或违法分包，在施工过程中，严格执行安全生产相关管理规定；按照广州市住房和城乡建设委员会的规定发包劳务或使用自有劳务队伍。

八、本公司如投标时选用绿色生产达标企业生产的混凝土，中标后将使用绿色生产达标企业生产的混凝土，若不遵守承诺，自愿按不履行合同约定处理。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
