5.1工程设计资质：工程设计综合甲级资质、或建筑工程设计行业丙级资质或以上、或建筑行业（建筑工程）专业设计丙级或以上、或建筑装饰工程设计专项甲级资质或相应的建筑装饰装修工程设计与施工一级资质（以资质证书许可范围为准，在有效期内）；

5.2工程施工资质：建筑装修装饰工程专业承包一级、或相应的建筑装饰装修工程设计与施工一级资质（以资质证书许可范围为准，在有效期内）。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>6.2投标人拟承担本工程设计负责人为：具备建筑工程专业（含相近专业）高级工程师。设计负责人与项目负责人不能为同一人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：（1）建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。根据广东省建设厅《关于明确省外二级建造师入粤注册和执业有关问题的通知》（粤建市函〔2011〕218号），二级建造师执业资格证书、注册证书仅限所在行政区域内有效，不得跨省执业。投标人提供有效的二级建造师执业资格证书或提供省建设执业资格注册中心“取得二级建筑师、二级结构工程师、二级建造师执业资格证书持证人员从业情况信息公开平台”上有效的《二级建造师执业资格证书持证人员从业情况信息公开证明》，《信息公开证明》提供纸质文件扫描件即可（适用于二级）。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>7、投标人自nth="1" Day="1" IsLunarDate="False" IsROCDate="False" w:st="on">2014年1月1日至今完成过质量合格的类似工程业绩（类似工程是指项目规模的中标金额大于或等于1300万元的建筑装修装饰施工工程，完成时间以广州市房建和市政企业库中项目业绩页面的“验收资料”栏中的竣工验收日期所记录的日期为准）。需同时提供中标通知书或免招标的相关证明、施工合同、竣工验收报告或竣工验收证明。如以上资料不能证明业绩规模的技术指标（指面积、高度、跨度、管径等）的，须另提供可证明业绩技术指标的其他资料。建筑装修装饰工程中标金额以广州市房建和市政企业库中项目业绩页面的“项目规模”栏中所记录的“建筑装饰装修工程”造价为准；完成时间以广州市房建和市政企业库中项目业绩页面的“验收材料”栏中的竣工验收日期所记录的日期为准；建筑装饰装修工程以在业绩库中“项目规模对应的资质”栏中登记有“建筑装修装饰工程”的为准。如业绩库中的“项目规模对应的资质”栏未登记有“建筑装修装饰工程”，或“项目规模”栏中所记录的“建筑装饰装修工程”造价小于1300万元，或登记的工程资质内容与施工合同等业绩证明资料不相符的，该项业绩无效。

联合体共同投标协议

广州市住房和城乡建设委员会、本招标人及招标监管机构：

本公司就参加                        投标工作，作出郑重声明：

一、本公司保证本次投标活动所提供的材料都是真实的；在广州市房建和市政企业库登记的一切信息都是真实的、在有效期内的。

二、资格审查前，投标人须建立企业诚信档案及拟担任本工程项目负责人须是本企业中的在册人员。

三、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

四、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；在投标报名截止日期前两年内没有建设行政主管部门已书面认定的重大工程质量问题；在广州市人民检察-院行贿犯罪档案查询结果中，本公司没有在投标报名截止时间前两年内被人民法院判决犯有行贿罪的记录。

五、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员。本公司严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

六、本公司及其有隶属关系的机构没有参加本项目的招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

七、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

八、本公司承诺，中标后按照《广州市建筑工程劳务分包管理办法（试行）》（穗建筑﹝2013﹞428号）的规定发包劳务或使用自有劳务队伍。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
