1. 招标条件
normal style="MARGIN: 0cm 0cm 0pt; LINE-HEIGHT: 20pt; mso-line-height-rule: exaly"> 本饶平县上饶镇埔中村省定贫困村创建社会主义新农村示范村工程项目（项目名称）已由 饶平县发展和改革局 以 饶发改【2017】252号 （批文名称及编号）批准建设，项目业主为 饶平县上饶镇人民政府 ，建设资金来自 贫困村创建社会主义新农村示范村建设资金 （资金来源），招标人为 饶平县上饶镇人民政府 。项目已具备招标条件，现对该项目施工采用资格后审进行公开招标。
2. 项目概况与招标范围
2.1 项目建设地点：饶平县上饶镇埔中村。
2.2 项目建设规模及内容：
①村内巷道硬化：地面硬化11026.5m2 ；
②生活垃圾处理：垃圾收集点建设4个；
③生活污水处理工程：一体化污水处理设施处理规模1800人，污水管网建设规模3000m；④集中供水工程：村庄供水管网升级3000m；
⑤建筑风貌提升工程：外立面整饰10000 m2；
⑥公共设施：公厕建设3个；沿溪路沿河整治长700m驳岸修复和150m护堤，栏杆修建，道路硬化，路灯照明；新建服务中心150m2，文体活动中心120m2，文体活动场地7360m2，亮化工程，路灯100个。
招标控制价为：1805.73万元。
2.3招标范围：依据河南省豫北水利勘测设计院有限公司设计的施工图纸工程内容、招标人提供的工程量清单所列工程内容。

3. 投标人资格要求

3.2企业主要负责人应具有行政主管部门颁发的安全生产考核合格证书（A证），项目负责人应具有行政主管部门颁发的安全生产考核合格证书（B证）或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的网页打印页，专职安全生产管理人员应具有行政主管部门颁发的安全生产考核合格证书（C证）或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的网页打印页；

normal style="TEXT-ALIGN: left; MARGIN: 0cm 0cm 0pt; LINE-HEIGHT: 17.3pt; TEXT-AUTOSPACE: ; TEXT-INDENT: 21pt; mso-line-height-rule: exaly; mso-char-indent-count: 2.0; mso-layout-grid-align: none" align=left>3.4拟投入项目架构的主要人员：①项目经理1人[具备二级或以上<省外企业应为一级>市政公用工程专业或建筑工程专业建造师（含临时证书），具有项目负责人安全生产考核合格证书（B类）或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的网页打印页]；②技术负责人1人(具有高级工程师或以上职称证书)；③市政工程师1名(具有市政公用工程相关中级工程师或以上职称证书)；④质量员不少于2人(具有相应的岗位证书)；⑤施工员不少于2人(具有相应的岗位证书)；⑥材料员不少于1人(具有相应的岗位证书)；⑦专职安全生产管理人员不少于1人(具有建设主管部门颁发的专职安全生产管理人员安全生产考核合格证书（C类）或提供广东省建筑施工企业管理人员安全生产考核信息系统相应人员信息资料的网页打印页，和相应的岗位证书)；⑧资料员不少于1人。上述人员在本项目只能担任一个岗位，不得兼任；相应的证明材料复印件[身份证、岗位证（职称证）或注册证书及近一年有效的社保证明材料复印件，社保证明以社保管理部门盖章并提供网站登录账号及密码、联系电话，报名时将网上核查]；
3.5投标人不能处于财产被接管、冻结、破产状态（提供由会计事务所审计的2014-2016年完整的财务审计报告）；
3.6出具拟投入本工程的项目负责人或法人代表参加当天开、评标会议的承诺书；
3.7投标人、企业法定代表人、项目负责人近3年来无行贿犯罪记录；
3.8投标人自2014年以来没有骗取中标和严重违约、经营中没有重大安全生产事故或质量事故、没有违法犯罪记录的承诺书；
3.9本项目接受联合体投标，但联合体成员最多不得超过2名。应以市政公用工程施工总承包三级（含三级）以上资质为主办方，并签定联合体工作协议。组成联合体投标的各法人单位均不得再以独立投标人身份参与本项目投标,也不得作为其他联合体的成员参与本项目的投标。
