1．招标条件

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">云浮市云安区大涌河治理工程已经云水建管【2017】21号文批准建设，招标人为云浮新区综合服务中心（云浮新区政府投资建设项目代建服务中心），资金来源除省、市按有关政策给予补助外，其余建设资金由云浮新区自筹解决,已具备招标条件，现对本项目的施工进行公开招标。

2.工程项目概况：

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">2.3建设规模及招标内容：本工程治理河道长度16.42km,其中大涌河干流6.85km，古洲河支流3.52km，洞坑新村支流0.61km，东山水支流2.924km，长迳河支流1.711km，长古头渠0.47km，长古头二渠0.33km。主要建设内容为：河道清淤疏浚16.42km，固脚护岸长度12.42km，新建排水涵8座，拆除重建交通桥9座。具体招标内容以工程施工图纸及施工预算书为准。

3．投标人资格要求:

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.1ink2">具有独立法人资格的施工企业，并符合招标文件要求的资格后审资格。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.2ink2">具备建设行政主管部门颁发的水利水电工程施工总承包叁级或以上资质，在报名前已到广东省水利厅完成信用信息录入审核手续并已公示终审通过。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.3ink2">具备建设行政主管部门颁发在有效期内的安全生产许可证。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.4ink2">拟担任本工程的项目负责人应具备水利水电工程专业二级或以上注册建造师资格或建造师临时执业资格或建造师执业资格证书持证人员从业情况信息公开证明；企业注册所在地不在广东省行政区域内的投标人的项目负责人必须具备水利水电工程专业一级建造师执业资格或建造师临时执业资格；且在本项目报名前已到广东省水利厅办理信用管理信息录入手续。项目负责人不可兼任于其他项目或其他岗位。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.6ink2">投标人的法定代表人、项目负责人、专职安全生产管理人员须取得水行政主管部门颁发的相应安全生产考核合格证书（即分别为：A证，B证，C证）。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.7ink2">施工员、质检员、安全员、材料员及资料员须具有水行政主管部门或中国水利工程协会颁发的对应岗位证书。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.8ink2">拟投入项目管理班子人员近3个月连续在投标人单位缴纳社保。各投标人可根据自身条件和工程实际情况增加和增派相关专业人员。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.9ink2">投标人在开标前到项目所在地或投标人所在地的人民检察-院进行贿赂犯罪档案查询，并持该院出具的投标人及法定代表人、拟担任本工程的项目负责人近三年（企业成立不足三年的，按成立之日起开始计算）无行贿犯罪记录的查询结果告知函证明文件才可参加投标。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.10ink2">投标人于2015年1月1日至本项目投标报名截止时间止须具有承接过一项单项合同工程造价为2000万元或以上的中小河流治理类施工项目业绩（注：提供公共资源交易中心签发的中标通知书）。

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">3.11ink2">本工程不接受联合体申请。

4. 踏勘现场

normal style="text-indent:21.2pt;mso-char-indent-count:2.02;line-height:20.0pt;mso-line-height-rule:exaly">4.1本工程要求全部项目管理机构人员施工期间常驻工程现场，管理机构人员签认驻场承诺前必须自行踏勘现场（踏勘所产生的一切费用自负，招标人可提供协助），充分了解工程风险、场地和周边环境情况。

normal style="text-indent:21.2pt;mso-char-indent-count:2.02;line-height:20.0pt;mso-line-height-rule:exaly">4.2拟参加投标企业的项目负责人、项目技术负责人、专职安全生产管理人员、施工员、质检员、材料员及资料员必须在2018年3月8日上午10时00分前，携带《踏勘现场证件查验表》一式二份，及其要求的相应原件，到达云浮新区综合服务中心（云浮新区政府投资建设项目代建服务中心）（云浮新区都杨镇开发区生活小区F栋一楼）登记，并自行前往现场踏勘（踏勘现场车辆及其它可能产生的一切费用，由拟参加投标企业自行负责）。潜在投标人现场踏勘完成后，需签认驻场承诺书《拟派项目管理人员驻场及项目负责人无在建项目承诺书》，然后持招标人或招标代理确认其已踏勘现场的有效回执和承诺书在规定的时间内参加报名。经登记确认的人员名单和资料在随后的报名及投标中不得更改。

5．报名、购买招标文件及图纸时间、地点：

normal style="text-indent:15.75pt;mso-char-indent-count:1.5;line-height:20.0pt;mso-line-height-rule:exaly">投标人可于2018年3月14日至2018年3月20日(法定公休日除外)，每天上午9:30-11:00、下午14:00-16:00由法定代表人或其授权代理人到广州公共资源交易中心（广州市天河区天润路333号）报名及购买招标文件，逾期恕不办理。招标文件工本费为3000.00元/合同段，售后不退，不购买者，投标无效。报名时需提交以下材料：

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">（5）拟派项目负责人：①广东省施工企业提交水利水电专业二级或以上的《建造师注册证书》或《建造师临时执业资格证书》或建造师执业资格证书持证人员从业情况信息公开证明；广东省外施工企业提交水利水电专业《一级建造师注册证书》且在本项目报名前已到广东省水利厅办理信用管理信息录入手续（含网址的网页截图）；②必须具有水利水电专业工程师或以上职称证书；③第二代身份证复印件；

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">（7）施工岗位证书：施工五大员（施工员、安全员、质检员、材料员及资料员）具有水行政主管部门或中国水利工程协会颁发的对应岗位证书及二代身份证；

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">（12）《拟派项目管理人员驻场及项目负责人无在建项目承诺书》。

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">（13）投标人于2015年1月1日至本项目投标报名截止时间止须具有承接过一项单项合同工程造价为2000万元或以上的中小河流治理类施工项目业绩（注：提供公共资源交易中心签发的中标通知书）。

normal style="text-indent:21.0pt;mso-char-indent-count:2.0;line-height:20.0pt;mso-line-height-rule:exaly">以上资料一式二份装订成册并逐页加盖公章，自行编写封面（注明项目名称、投标人名称、单位地址、单位电话、联系人、联系电话），以上所有证件均提供原件核查（除第10项材料外，凡能通过二维码扫描查证的证书除外）。所提供的证书、资料必须完整真实，报名时的人员名单必须与踏勘时的人员名单一致。投标人如不按要求提交报名资料或提交报名资料不全，或提交报名资料复印件与原件核对不一致，招标人则不予接受报名。凡弄虚作假骗取中标的，一经查实，将取消中标资格并向行政主管部门以予以通报。
