normal style='text-indent:21.0pt;mso-char-indent-count:2.0;line-height:150%'>四、项目概况、建设规模：“广东省禁毒科普馆”设计与制作一体：项目总建筑面积2800平方米，其中展览教育用房2125平方米、公众服务和业务研究用房460平方米、管理保障用房215平方米。建设内容包括建筑工程、安装工程、平面展陈及室外工程。其中设计包括方案设计、初步设计、施工图设计（含预算）、竣工图编制和相关服务；设计单位需综合考虑设备选型的预留预埋，后续设备采购中的多媒体点位布线及所有多媒体等技术设备的选型及技术参数。施工包括施工图范围内的全部内容施工、原展厅拆除（可利用设备的保护性撤除，展柜专业可再利用性拆除及保存）设备材料采购、安装和保修，含展厅基本装饰装修（包括弱电智能化系统、电气工程以及与装饰装修和陈列布展有关的安防消防空调强弱电系统调整、补充、完善等）和布展（包括展台、展板、多媒体、各种辅助性实体模型、艺术品制作等）。配套工程计划完成展厅拆除及修复、排水系统升级、防水工程升级、展厅通风系统升级（含空调）、布展消防配套工程及外立面装饰等。包括但不限于以下内容（具体内容详见合同协议书）。

5、投标人需同时具备承接本工程所需的设计类和施工类资质：

5.1工程设计资质：工程设计综合甲级，或建筑行业设计丙级或以上资质，或建筑行业（建筑工程）专业设计丙级或以上资质，或建筑设计事务所资质；

5.3施工资质：建筑工程施工总承包三级或以上资质；

6、投标人拟担任本工程项目负责人及设计负责人：

6.1项目负责人（兼施工负责人，下同）的人员为：建筑工程专业二级或以上的注册建造师；投标人在报名时由广州公共资源交易中心对投标人该项目负责人进行锁定。

6.2设计负责人的人员为：须具备高级工程师或以上资格；设计负责人与项目负责人不能为同一人。

normal style='text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly'>注：建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

7、项目负责人须持有在有效期内的安全生产考核合格证书（B类），或能够提供广东省建筑施工企业管理人员安全生产考核信息系统安全生产管理人员证书信息的打印页。

normal style='text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly;background:white'>10.1允许联合体投标，但只接受最多由2家单位（即一家设计单位、一家施工单位）组成的联合体，应以承担施工任务的一方为主办方，并签定联合体共同投标协议书。联合体共同投标协议书应明确约定各方拟承担的工作和责任。

normal style='text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly'>10.3联合体的资格条件按联合体任务分工进行评审（其中建筑工程施工总承包三级或以上资质、安全生产许可证、项目负责人、专职安全人员、类似项目施工业绩以承接施工任务的主办方为准；工程设计综合甲级，或建筑行业设计丙级或以上资质，或建筑行业（建筑工程）设计丙级或以上资质，或建筑设计事务所资质、设计负责人以承接设计任务的成员单位为准））。

normal style='text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly;background:white'>11、资格审查前，投标人（含联合体各方）须在广州市城乡建设委员会建立企业诚信档案，拟担任本工程项目负责人是本企业诚信档案中的在册人员。（联合体投标的，诚信备案和人员在册情况为联合体协议分工对应企业库内诚信档案）。

normal style='text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly'>十、资格审查方式：本项目采用资格后审，由评标委员会负责对投标人的资格进行审查，评标时只对通过资格审查的投标人进行下一阶段的评审。满足资格审查合格条件的投标人不足3名或经评审通过设计部分（或施工部分）有效性审查的投标人少于3名时为招标失败。招标人分析招标失败原因，修正招标方案，报有关管理部门核准后，重新组织招标。本项目为非电子评标，投标人需按招标文件要求提交纸质文件及电子光盘。（信息取自广州市房建和市政企业库的除外）

normal align=left style='text-align:left;text-indent:23.5pt;mso-char-indent-count:2.24;line-height:26.0pt;mso-line-height-rule:exactly;mso-pagination:widow-orphan;background:white'>十二、投标申请人投标时应按附件一《资格审查书面资料及其他取自广州市房建和市政企业库内资料一览表》中提交资料形式的要求编制投标文件中的资格审查文件。企业库记录的该部分上传件将被视为投标申请人递交资格审查资料的一部分。资格审查委员会对该部分的审查将以递交投标文件截止时间时在广州市房建和市政企业库内的上传件为依据。若截标后，开标时间因故推迟的，相关评标信息仍以原递交投标文件截止时间的信息为准，且投标人应及时维护相关上传件，确保各项上传件在有效期内。

normal align=left style='text-align:left;text-indent:21.0pt;mso-char-indent-count:2.0;line-height:26.0pt;mso-line-height-rule:exactly;mso-pagination:widow-orphan;tab-stops:5.25pt;background:white'>2、设计费由投标人根据招标文件要求以及企业自身情况填报。投标人根据可行性研究报告的方案及要求进行设计，参照国家发展计划委员会、建设部2002年颁布的《工程勘察设计收费标准》相关规定，投标的设计方案总造价不得超过投资限额。在合同实施期间设计费不予调整方案深化设计、施工图设计占全部设计工作量比例为100 %。

normal align=left style='text-align:left;text-indent:21.0pt;mso-char-indent-count:2.0;line-height:26.0pt;mso-line-height-rule:exactly;mso-pagination:widow-orphan;tab-stops:5.25pt;background:white'>3、施工费用由投标人根据招标文件要求以及企业自身情况填报下浮率，投标报价不得高于施工费的招标控制价。投标人根据招标文件要求以及自行设计方案编制投标报价。中标后，中标单位根据经审定的方案进行方案深化设计、施工图设计，并按国家、广东省、广州市以及现行有关工程造价管理的文件编制施工图预算，报送招标人进行审核，审定的施工图预算按合同专用条款第23条调整后作为施工进度款支付及结算的依据。送审施工图预算不得高于建安费施工投标最高投标限价。

本招标项目招标人及招标监管机构：

本公司就参加广东省禁毒科普馆改扩建项目工程设计施工总承包投标工作，作出郑重声明：

一、本公司保证投标报名材料及其后提供的一切材料都是真实的。如我司成为本项目中标候选人，我司同意并授权招标人将我司投标文件商务部分的人员、业绩、奖项等资料进行公示。

二、本公司保证在本项目投标中不与其他单位围标、串标，不出让投标资格，不向招标人或评标委员会成员行贿。

三、本公司没有处于被责令停业的状态；没有处于被建设行政主管部门取消投标资格的处罚期内；没有处于财产被接管、冻结、破产的状态；本公司没有被人民法院列入失信被执行人名单。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

八、本公司如投标时选用绿色生产达标企业生产的混凝土，中标后将使用绿色生产达标企业生产的混凝土，若不遵守承诺，自愿按不履行合同约定处理。

本公司违反上述保证，或本声明陈述与事实不符，经查实，本公司愿意接受公开通报，承担由此带来的法律后果，并自愿停止参加广州市行政辖区内的招标投标活动三个月。
