normal style='text-indent:24.0pt;line-height:150%;word-break:break-all'>1、本次询比价为整体采购，询比价响应供应商报价时须写明单价及总价、产品的详细配置参数，报价包含货物制造、运输、装卸、售后服务等交付采购人使用前所有可能发生的费用，确定成交供应商不再增补任何费用。

normal style='text-indent:24.0pt;line-height:150%;word-break:break-all'>3、交货地点：上海市浦东新区航都路80号

normal style='text-indent:24.0pt;line-height:150%;word-break:break-all'>4、质量标准或要求：按技术协议以及框架合同

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（1）报价方是制造企业的，应是在在中国境内（含港澳台地区）/外登记注册的独立法人，具有独立承担民事责任的能力，注册资本或生产能力要求，具有有效期内的ISO9001质量管理体系认证并具有完善的质量管理体系和其他资质证书。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（2）报价方是代理商的，应是在中国境内（含港澳台地区）登记注册的独立法人，具有独立承担民事责任的能力，所代理的制造企业满足第1条中相关要求，并具有生产制造企业出具的针对本招标项目的唯一授权书。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>制造企业及其代理商不得同时参加本次投标。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（3）报价方是增值税一般纳税人，能开具增值税专用发票。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（4）报价方近3年具有与本次招标产品相同或相近技术能力设备销售业绩不少于5个。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（5）具有良好的银行资信和商业信誉，近三年没有处于被责令停业，财产被接管、冻结、破产状态。

normal style='text-indent:21.0pt;line-height:150%;word-break:break-all'>（7）报价方不存在《中华人民共和国招标投标法》、《中华人民共和国招标投标法实施条例》禁止投标的情形。
