normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%;layout-grid-mode:char'>5、投标人自2011年1月1日至今完成过2个及以上质量合格且容量不低于35t/h的电力或热力工程建设项目（含土建施工、设备安装、调试）监理业绩。需同时提供监理合同（包含签字盖章页、表明监理范围以及项目概括的页）、竣工验收报告(或证明) 复印件及业绩所属业主名称、地址、联系人及联系电话。
