投标人同时具有承接本工程所需的以下①、②、③所述专业承包资质：

①建筑装修装饰工程专业承包贰级（或以上）资质或相应的在有效期内的建筑装饰装修工程设计与施工资质（以资质证书许可范围为准）；②特种工程（仅限结构补强）专业承包资质；③建筑机电安装工程专业叁级（或以上）资质。

投标人拟担任本工程项目负责人的人员为：建筑工程专业壹级注册建造师。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>注：①招标内容含有设计要求，且设计要求仅为深化设计的，在投标人的资质设置要求中，不允许设置设计资质要求。建造师的专业及等级标准按《注册建造师执业管理办法（试行）》及《注册建造师执业工程规模标准（试行）》；注册建造师包括延续注册成功的注册临时建造师。项目负责人在任职期间不得担任专职安全员，项目专职安全员在任职期间也不得担任项目负责人。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>如投标人组成联合体，应以承接 建筑装修装饰工程 的一方为主办方，并签订联合体工作协议。投标人拟任本工程项目负责人应为主办方正式员工。联合体工作协议应明确约定各方拟承担的工作和责任。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>（一）组成联合体承接按资质标准划分为同一类工程的投标人，在资格审查环节，其资质、业绩、信誉等指标按联合体成员中最低者确定；在择优和评标环节，其业绩、信誉、诚信综合评价分数等指标均按成员中最低者进行评审，其人员、资金、机械设备等资源性指标可合并计算。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>（二）组成联合体承接按资质标准划分为多类工程的投标人，应在联合体协议中明确承接每一类工程的成员单位。在资格审查、择优和评标环节，均应先按本条第（一）项规定逐类评审，再将各类评审结果汇总，各投标人诚信分以招标控制价中各类工程的占比作为权重，将其各类诚信分加权平均计算。

normal style='text-indent:26.9pt;mso-char-indent-count:2.24;line-height:150%'>9、资格审查前，投标人（含联合体各方）须在广州市住房和城乡建设委员会建立企业诚信档案及拟担任本工程项目负责人须是投标人（或联合体主办方）诚信档案中的在册人员。

注：1、投标人nEstyle='font-size:12.0pt;line-height:150%;font-family:宋体;mso-ascii-font-family:"Times New Roman";mso-hansi-font-family:"Times New Roman"'>按招标文件11.2的要求提交资料。若资料取自广州市房建和市政企业库的，企业库内的该部分上传件将被视为投标申请人递交资格审查资料的一部分。评标委员会对该部分的审查将以递交投标文件截止时在广州市房建和市政企业库内的上传件为依据。若招标人延长递交投标文件时间，以延长递交投标文件时在广州市房建和市政企业库内的上传件为依据。投标人应及时维护相关上传件，确保各项上传件的有效性。

2、未在招标公告第十一条单列的资审合格条件，不作为资审不合格的依据。

四、本公司拟委派专职安全员兼任本工程的工地余泥渣土运输与排放管理员，严格遵守建设工程余泥渣土运输与排放管理制度，执行“一不准进、三不准出”规定，选择合法的余泥渣土运输单位及排放点。承诺如违反建设工程余泥渣土运输与排放管理制度，将自愿接受：通报批评，记录不良行为，列入黑名单，并暂停责任企业投标报名一年，对责任项目负责人暂停投标报名二年。多次违规的，暂停投标报名二至三年，并提请资质审批部门降低或吊销企业资质、项目经理的建造师从业资格和专职安全员安全培训考核证书。

五、本公司及其有隶属关系的机构没有参加本项目的设计、前期工作、招标文件编写、监理工作；本公司与承担本监理业务的单位没有隶属关系或其他利害关系。

六、本公司保证本项目拟派的项目负责人和安全员没有在其他在建项目中任职。

七、本公司承诺，中标后不转包或违法分包，在施工过程中，严格执行安全生产相关管理规定；按照广州市住房和城乡建设委员会的规定发包劳务或使用自有劳务队伍。
